Dr. Cummings is a graduate of the University of Health Sciences, College of Osteopathic Medicine in Kansas City, Missouri. Upon his graduation from medical school Dr. Cummings did his internship at the Lakeview Hospital, Milwaukee, WI and served as Chief Resident at Niagra Falls Memorial Medical Center. Dr. Cummings is board certified in Family Practice and is a Fellow of The American Academy of Family Physicians. He is the former medical director at Forest Farm Nursing and Rehabilitation Center in Middletown, RI. In addition, he served on staff for 25 years at St. Anne's Hospital in Fall River, MA. Dr. Cummings also previously served as the interim Medical Director at Health First, a community Health Center in Fall River. He now focuses his time just on outpatient Family Practice with PrimacCare.

Cody Zygmont MSN, NP-C

Family Nurse Practitioner - Board Certified

Cody recieved his BSN from The University of Massachusetts Dartmouth and worked at St.Anne's Hospital in Fall River, MA as a registered nurse for 5 years. After graduating from Simmons College with his MSN he became a board certified Family Nurse Practitioner and continue to serve the community at Tiverton Primary Care Associates, Prima CARE Walk-in, and a number of Nursing Centers throughout the Fall River area.
